Personal Support Worker

Job Description

RESPONSIBILITIES:

  • Provision of comprehensive care in an interdisciplinary team
  • Provide continuous and comprehensive support to your team in the delivery of care for your clients 
  • Maintains effective relationships/communication and works collaboratively with other disciplines to implement interdisciplinary care plans within your role
  • Participate in regular interdisciplinary team communication (e.g. huddle, rounds, status updates)
  • Use the TAMVOES communication platform to effectively communicate changes in the client’s condition to the primary care lead/clinician and other members of the care team as required
  • Assists clients in the activities of personal care, instrumental activities of daily living (IADLs) functional mobility and other activities outlined in the client’s care plan.
  • Provision of delegated acts/treatments as part of the interdisciplinary team and within the role of an unregulated care provider
  • Provide care with clients and families adhering to Health’s policies and procedures as adopted from Ministry of Health Guidelines
  • Proactively identifies, promotes and seeks to improve client safety as part of their everyday work

REQUIREMENTS:

  • A PSW Certificate is required
  • One year of relevant experience is preferred
  • Community experience is an asset.
  • Vulnerable sector check (current).
  • Current CPR and First Aid Certificate is required
  • A knowledge of household management skills is required
  • A working knowledge of assistive aids is required
  • Ability to communicate effectively (oral and written) and exercise good judgement is required
  • Ability to lift up to 25 kilograms may be required
  • Ability to perform lifting and bending in awkward positions when assisting clients may be required
  • Ability to use communication tools is required
  • Demonstrated skills working independently and in a team environment
  • A vehicle and valid driver’s license and the ability to travel
  • Available for shifts and visiting clients in their homes
  • A background check is required

Care Coordinator

Job Description

TAMCARE Home Health Ltd. is seeking a dedicated care coordinator to assist us with patients enrolled at our facility. You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, and working with the care team to evaluate interventions.

The successful applicant will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.

Responsibilities:

  • Consult with patients and family members to discuss their health problems
  • Educate patients about their condition, medication, and give them specific instructions
  • Develop a care plan to address their personal health care needs
  • Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans
  • Check-in on the patient regularly and evaluate and document their progress
  • Assist the care team with developing and assessing health interventions
  • Attend ongoing training and courses to keep abreast of new developments in health care
  • Assist with securing funding for medical care as required
  • Treat patients with empathy and respect and conduct oneself in a professional manner
  • Comply with organizational guidelines and health care laws and regulations

Requirements:

  • A BA/BS in Social Work, or a related field
  • Licence may be required
  • At least 2-3 years’ experience in care coordination or clinical practices
  • Good verbal and written communication skills
  • Computer skills and proficiency in Microsoft Office
  • Strong analytical thinking and the ability to handle multiple tasks concurrently

Housekeeper

Job Description

TAMCARE Home Health Ltd. is seeking a dedicated housekeeper to assist us with patients enrolled with our company. You will be responsible for providing light housekeeping and assistance duties within the homes of our patients. This role is essential for our company as it will help us to provide a difference in the lives of our clients.

The successful applicant will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.

Responsibilities:

  • Cleaning the resident’s home and living spaces
  • Providing laundry services for the client
  • Washing clients’ dishes and other services around the kitchen
  • Cook meals for the client
  • Pick up supplies and necessities for the patients as needed from other locations
  • Work with the care team to prioritize tasks

Requirements:

  • Grade 12 or equivalent 
  • Ability to read, write, and speak English fluently
  • Previous healthcare housekeeping experience or a strong employment reference
  • Good understanding of healthcare infection control practices and procedures
  • Ability to use commercial cleaning products and equipment
  • Ability to set/adjust priorities and organize work in response to hospice needs
  • Ability to cook simple meals
  • Ability to communicate effectively with residents, families, and hospice staff both verbally and written
  • Background check in required
  • Ability to lift up to 50lbs
  • Access to a vehicle and the ability to travel is a definite asset
